GP Referral Service
Who will benefit?
If you are a little overweight or not as fit as you'd like to be, you would benefit from our 12 week course of exercise at our Leisure Centre.
Exercise can also play a key role in assisting post-operative recovery, mobility, insomnia, back pain and weight loss, as well as improving self esteem and fitness. In addition, research has proven that regular exercise can improve a broad range of physical and psychological medical conditions including, amongst others;
- Angina
- Diabetes
- Arthritis
- Obesity
- Hypertension
- Asthma
- Anxiety
- Depression.
How the GP Referral Scheme Operates:
Referral
A local GP, Practice Nurse, Consultant or other Medical Professional refers patients by issuing them with an 'Activity Referral' Form. This referral gives details of any medical problems the patient may have which will need consideration when the RPC is formulating an appropriate course of exercise. We will supply health professionals with the 'Activity Referral' Forms.
Welcome Session
The patient's first visit to our Leisure Centre will be in the form of a 'Welcome Session'. This will give them the opportunity to meet their RPC and become familiar with the facilities. The RPC will then arrange a confidential consultation where the medical needs and preferences of the patient will be taken into account in the formulation of the 12 week exercise programme. At this consultation the patient will undergo their first fitness assessment - this test is repeated after 6 and 12 weeks to assess the patient's improvement.
Exercise Programmes
The patient will be given an attendance card to record their visits and activities over the 12 week period. Our RPC will be happy to go through the different activities that will be available to you.
Cost
Discounted rates will be offered to patients on the Programme and further incentives will be given to encourage them to continue exercising after their 12 week course has been completed.
